- Varicella (Chicken Pox)
- Hepatitis B
- Tuberculosis (TB) and Annual Student Health Form (see information link at the bottom of the page)
- CPR Certification BLS or ACLS fromÌý /Military
- Tetanus
- Annual Influenza vaccine
